IUI – intrauterine insemination – is often the first fertility treatment I recommend to couples. It is effective, affordable, and fertilisation happens naturally inside your body. Many people think of IUI as less than IVF. I think of it as the right treatment for the right person.

IUI means we take your partner’s (or a donor’s) sperm, wash and concentrate it in the lab to select the healthiest swimmers, and then place them directly into your uterus at exactly the right time. Think of it as precision-assisted natural conception. We remove barriers such as poor timing, cervical mucus issues, or lower sperm counts and give sperm the best possible head start.

Your IUI Journey – Every Step Explained

Step 1: Initial Consultation and Assessment

We will review your fertility history, your partner’s, and decide together whether IUI is the right path. I only order tests that are clinically necessary for YOUR case.

Step 2: Ovulation Monitoring or Stimulation

Depending on your cycle, we will either track your natural ovulation or use gentle oral medications to encourage follicle growth. I monitor you closely to find the perfect timing.

Step 3: Sperm Collection and Preparation

On IUI day, your partner provides a fresh sample (or we thaw donor sperm). In the lab, we wash the sperm – removing seminal fluid and concentrating the healthiest, most motile sperm. The collection room is completely private.

Step 4: The IUI Procedure

This is the part patients are most nervous about – and the part that is easiest. It takes about 5 minutes, feels like a Pap smear or less, and needs no anaesthesia. I will talk you through every step as it happens.

Step 5: The Two-Week Wait and Result

Most patients describe it as mild discomfort, similar to a Pap smear or cervical swab. The catheter insertion causes brief cramping in some people. It lasts about 5 minutes total. No anaesthesia is required, and you can drive yourself home.

The general guideline is 3-4 well-timed cycles. If 3 cycles have failed with good sperm and confirmed ovulation, the additional benefit of more IUI cycles is usually small. I will review your specific results after each cycle and give you an honest recommendation on whether to continue or move to IVF.

Our IUI success rate is 25-30% per cycle – significantly above the national average of 15-18%. Success depends heavily on age, the reason for infertility, sperm quality, and timing. Cumulative success over 3 cycles reaches 50-70% for well-selected candidates.

Not always. Natural cycle IUI requires no stimulation medications at all – just monitoring. Oral medications (Letrozole or Clomiphene) are used for mild stimulation. Injectable gonadotropins are only used when oral medications are insufficient. We always start with the simplest, least invasive protocol appropriate for your situation.

IUI can work with mild male factor infertility – sperm washing concentrates and selects the best sperm, compensating for lower counts. However, if the post-wash count is below 5 million progressively motile sperm, IUI success rates drop significantly and IVF with ICSI is usually the better recommendation.
