  • Step 2 — Hormonal Profile: FSH (sperm production), LH (testicular function), Testosterone (affects sperm and libido), Prolactin (elevated levels suppress production), Thyroid function (impacts male fertility too).
  • Step 3 — Scrotal Ultrasound: Non-invasive scan that detects varicocele (most common reversible cause), evaluates testicular size and texture, identifies cysts, structural abnormalities, and obstructions. Quick and painless — 15-20 minutes.
  • Step 4 — Genetic Testing (When Indicated): Karyotype for chromosomal abnormalities, Y-chromosome microdeletion for severe oligospermia, CFTR gene testing for congenital absence of vas deferens. Not routine — only when clinically indicated.
  • Step 5 — Sperm DNA Fragmentation: The test most clinics skip. High fragmentation can cause failed fertilisation, poor embryo development, and recurrent miscarriages — even with normal basic parameters. Treatable with antioxidants and MACS technique.
  • Step 6 — Advanced Sperm Function Tests (When Needed): Reactive Oxygen Species (ROS) testing for oxidative stress, anti-sperm antibody testing for immune factors, and hypo-osmotic swelling test for membrane integrity.
